If you are interested in becoming a new client, please contact me directly through the "Contact Today" page. Although the process of being added onto my client roster is not incredibly difficult, availability can be limited and I am not always accepting new clients. Once you and I have confirmed that I can add you to my schedule, the intake process will go as described below.
I prefer speaking directly with you to better understand your concerns for seeking counseling. Once we decide to work together, and your appointment has been finalized, I will send you an email link to create a new client profile on a secure website "client portal" where you will start the process. Registration documents are typically completed online in an attempt to keep my office relatively paperless.
I recommend that forms be completed thoroughly and in a timely manner. Please submit all of the intake forms at least 24 hours before our scheduled appointment. This allows me the time needed to review your information properly before your initial session.
Couple/Partner registration forms require information to be filled out by both individuals. I suggest you complete the registration forms together, if possible, so there is balanced input from each participant. After talking with you, I will email a link that allows you to complete the registration documents online by creating a client portal account.
All new clients are required to sign a Consent to Treat form as required by HIPAA. This also serves as a written agreement between client and counselor. I strongly recommend you read the Consent to Treat thoroughly and ask questions during the first session. You're welcome to call, text, or send me an email with any questions you have beforehand. This form requires your (electronic) signature. The Consent to Treat is included with documents you will have access to via the client portal. Access to the client portal will be granted to you after first speaking with me and setting up the initial appointment.
The Notice of Privacy Practices document goes into detail about how personal information about a client may be used and disclosed, as well as how clients may get access to their records. It is kept in your file showing that you understand HIPAA and that you were provided the information about it and will require your (electronic) signature.
